Club Horizon, Inc. is a non-profit organization committed to providing services to improve the quality of life for adults with mental illness.  Club Horizon provides Community Support services to adults with major mental illness such as schizophrenia, schizoaffective disorder, bi-polar disorder, major depression, anxiety disorders, substance related and mood disorders. 

What is Community Support?

Club Horizon Community Support services are a combination of case management and one on one support services designed to assist adult mental health consumers who need assistance in the following areas: self-care, peer relationships, family relationships, jobs, school or primary health needs.  These consumers may have difficulty accessing services, need crisis intervention, may be at risk for hospitalization, or may have difficulty finding a doctor, a job, a therapist, or housing. 

Club Horizon Community Support services consist of mental health and substance abuse services/supports necessary to assist the consumer in achieving and maintaining independent living, sobriety, and recovery goals.  Community Support interventions include assistance accessing benefits, money management, locating affordable/appropriate housing, symptom monitoring, medication monitoring, coping skills, social skills, assistance with employment, or continuing education.  Community Support services also include locating a therapist, finding a day program, managing leisure time, development of communication skills and symptom management. 

How is Community Support Provided?

Each consumer is assigned a Community Support Professional who will coordinate mental health, substance abuse, social and financial services, directly to the person and their family.  Club Horizon will serve as the clinical home for the consumer and assure that coordination of services includes therapists, psychiatrists, day programs, and other providers as identified by the consumer. 

Service will begin with an assessment of consumer needs and personal goals.  All goals and strategies will be developed with the consumer and significant others as identified by the consumer.  Each plan will be individualized to meet the consumers unique set of challenges and to build on personal strengths.

Community support will also include arranging, linking, and monitoring services for the consumer.  Community Support staff will continuously assess the needs of the consumer to assist in achieving the highest level of independent functioning at home and in the community.  Community Support is provided in the community – the consumer’s home, grocery store, doctor’s office bank or wherever support is needed.  Crisis Response is available 24/7/365.  Club Horizon is committed to consumer and family driven service provision.
